This editorial describes the urgent need for a global response to the mpox outbreak, which has been declared a Public Health Emergency of International Concern. The outbreak involves a new strain with higher fatality rates, affecting mainly children in DR Congo. Over 16,000 cases and 500 deaths have been reported. Immediate actions include improving health infrastructure, training workers, and ensuring vaccine access. Past neglect of mpox is criticized as few vaccines are available and some treatments are ineffective against this strain. Equity in response is crucial to prevent another unequal scramble for resources like during COVID-19.
