This article discusses the development of a Criteria Outcomes Tool (COT) for validating the effectiveness of a mass casualty incident (MCI) algorithm in correctly triaging pediatric trauma patients. The background emphasizes the importance of accurate triage to prevent over-triage and under-triage in MCIs. The COT, consisting of 47 outcomes and interventions, categorizes patients based on resource utilization and clinical outcomes. Through an anatomic approach and a modified-Delphi method, the COT was formulated and reviewed by experts. Applying the COT to a cohort of 25 pediatric victims showed 100% agreement between reviewers for Red, Yellow, Green, and Black triage outcomes, indicating good reproducibility.
